Tiers of Care

Care that grows with your loved one.

Memory loss isn't one diagnosis at one moment — it's a long, slow becoming. EverCare's in-home dementia care is built in three thoughtful tiers across Macomb, Oakland, and Wayne counties, so the right amount of help shows up at the right time, scales up the day life changes, and never asks your family to switch agencies, learn new faces, or start over.

EverCare offers three transparent tiers of in-home memory care across Metro Detroit: Companion Care for early-stage support, Memory Care Essentials for daily mid-stage care, and Comprehensive 24/7 in-home care for advanced needs. All tiers use the same trained caregiver model, hourly pricing, and no long contracts.

How the tiers work

What are the three tiers of EverCare's in-home memory care?

Most families start at Tier 1 or 2 and move up only when they need to. There are no contracts, no minimum hours, and no penalty for scaling down on a good week. The same care coordinator and the same primary caregiver follow your loved one through every tier — so your family is never starting over with a stranger.

Tier 1

Companion Care

Early-stage & preventative support

For families noticing the small things stack up — skipped meals, missed medications, mail piling on the counter, long lonely afternoons, the same story told three times in an hour. Companion Care places a familiar, dementia-aware caregiver in the home before a crisis forces the decision. We help your loved one stay independent longer, keep routines steady, and give the family room to breathe — without rushing into a higher level of care a moment before it's needed.

  • Friendly visits, conversation, reminiscence, and gentle cognitive activity
  • Medication reminders, hydration tracking, and home-cooked nutritious meals
  • Light housekeeping, laundry, linens, and tidy-up of high-use spaces
  • Grocery shopping, prescription pickup, and errands run on your loved one's behalf
  • Safe transportation to medical appointments, salon visits, and social outings
  • Home safety walkthrough on every visit (rugs, stairs, stove, locks, water temp)
  • Detailed family update after every visit — what we noticed, what changed

Memory Care Essentials

Mid-stage daily dementia support

When one family member can't carry it all anymore — when bathing becomes a battle, sundowning starts at 4 p.m., and the calls from the neighbor are getting more frequent — Memory Care Essentials is the daily, dementia-trained help that holds the day together. Same primary caregiver every visit (we don't rotate strangers through your loved one's living room), structured routines built specifically for Alzheimer's and related dementias, and behavior support strategies that actually de-escalate instead of medicating around the problem.

  • Same primary caregiver every visit + one trained backup who knows the care plan
  • Hands-on personal care: bathing, dressing, grooming, toileting, and incontinence support
  • Sundowning, agitation, exit-seeking, and anxiety management using validation techniques
  • Calm, predictable daily routine designed around your loved one's life history and preferences
  • Nutritious meal prep with attention to chewing, swallowing, and appetite changes
  • Mobility support, fall prevention, and safe transfers with proper body mechanics
  • Detailed written + verbal family update after every shift — mood, intake, output, behaviors
  • Dedicated care coordinator who knows your case by name and answers when you call

Tier 3

Comprehensive 24/7 Care

Advanced dementia, post-hospital & end-of-life

Round-the-clock coverage so no one in the family has to be the one on call at 3 a.m. Built for advanced dementia, late-stage Alzheimer's, post-hospital recovery, Parkinson's-related cognitive decline, and the families who have decided their loved one will age — and pass — at home, with dignity, in a familiar bed. We coordinate with hospice, palliative teams, hospital discharge planners, and primary physicians so the medical side runs smoothly and the family can return to being family.

  • Awake overnight, 24-hour, or live-in coverage with seamless caregiver hand-offs
  • A small, dedicated team of 2–4 caregivers — never a rotating roster of strangers
  • Two-person transfers, Hoyer lift support, and advanced mobility/positioning care
  • Skin integrity, repositioning schedules, and pressure-injury prevention
  • Hospice and palliative care coordination, comfort care, and end-of-life vigil support
  • Post-hospital recovery: discharge follow-through, readmission prevention, wound watch
  • Direct line to a care coordinator, 24 hours a day, 7 days a week — including holidays
  • Family respite blocks built in so primary caregivers can sleep, travel, and recover

How do I know which care tier my loved one needs today?

Choosing a level of in-home memory care is rarely a single decision — it's a conversation that unfolds over days. Most families land on Tier 2 (Memory Care Essentials) because that's the point at which one person at home can no longer manage personal care, sundowning, and safety on their own. Tier 1 fits earlier — when independence is mostly intact but loneliness, medication errors, and missed meals are starting to show. Tier 3 is for the season when 24-hour eyes are required: after a hospital stay, in advanced dementia, or when a family chooses hospice at home instead of a facility.

You don't have to figure this out alone. Take our short Care Fit Quiz or schedule a free in-home assessment. A care coordinator will sit at your kitchen table, listen, and recommend a tier — with no pressure and no sales pitch.

What's included in every EverCare in-home memory care tier?

  • Direct text + phone line to your care coordinator (not a call center)

Frequently asked

Tiers, pricing, and the fine print.

Can we change tiers as my loved one's needs evolve?

Yes — and most families do. Dementia isn't a static condition; needs shift over months or sometimes overnight after a fall or hospitalization. We review your care plan every 30 days and recommend tier changes proactively, but you can scale up or down anytime. Move from a few weekly companion visits to daily personal care, or to round-the-clock support, without paperwork or penalty. The whole point of our tiered model is that the care follows the person — not the other way around.

How quickly can care begin?

If you call before noon on a weekday, same-day care is usually possible. Otherwise, we typically have a caregiver in the home within 24 to 48 hours of your free in-home assessment. Hospital discharges, falls, and overnight crises don't follow business hours, so neither do we. A real person answers the phone 24/7 at (586) 326-3256 — including weekends and holidays — and our care coordinators move at the pace your family actually needs, not the pace a corporate process would prefer.

Is there a long-term contract?

No. EverCare runs on a simple month-to-month agreement. You can pause, scale up, scale down, or cancel with 48 hours' notice — no penalty, no exit fee, no awkward conversation. We earn your business every week. We've watched too many families get locked into 20-hour weekly minimums they didn't need, and we built EverCare specifically to be the opposite of that.

Do you accept long-term care insurance?

Yes. Most long-term care insurance policies cover the kind of in-home companion, personal, and dementia care we provide. We help families read their policy, understand the elimination period, and submit the claim documentation — including the daily care notes most insurers require for reimbursement. We also work with families using Veterans Aid & Attendance benefits. Bring your policy to the free assessment and we'll walk through what's covered before you commit to any hours.

What's included in the free in-home assessment?

A senior member of our team comes to your loved one's home for a calm, no-pressure 60-minute conversation. We meet the person, not just the diagnosis — noticing the layout, the routines, the worries no one's said out loud. We assess cognitive stage, fall risk, medication patterns, nutrition, and family caregiver burnout. Within 24 hours you receive a written care plan with recommended tier, hours, focus areas, and a flat hourly rate. There is no obligation and no follow-up sales call.

Can we add or remove hours week-to-week?

Absolutely — that flexibility is the whole point. Need extra coverage during a family wedding, a hospital stay, or a tough sundowning week? We add hours on request. Need to scale back during a quiet stretch or when out-of-town family is visiting? Just tell your care coordinator. There are no minimum-hour packages and no penalties for adjusting. Your bill reflects exactly the care delivered — nothing more, nothing less.
